Outreach is a specialist service designed to provide you with support in your home and in the community if you have mental health needs or learning disabilities. You are supported to live as independently as possible with flexible support dependent on your individual needs and ranging from 1 to 24 hrs a day.
At S&P Care we believe that every person deserves the chance to achieve their fullest potential and we encourage positive risk taking to help you maximise your independence and control of your life; to build resilience; and where needed, to build on your recovery and rehabilitation. Our team will work with you and any other professionals involved in your care to agree on goals and outcomes that we will support you to work towards.
