Who This Service Is For
This service is suitable for individuals who:
Live with respiratory conditions such as COPD or other chronic lung diseases
Require oxygen therapy or respiratory monitoring at home
Experience breathlessness or reduced mobility due to respiratory issues
Have recently been discharged from hospital following a respiratory-related admission
Require ongoing support to manage symptoms and maintain stability
What Support May Include
Respiratory & Clinical Support
- Support with oxygen therapy (as prescribed)
- Monitoring of breathing, oxygen levels, and general condition
- Recognising early signs of deterioration
- Supporting positioning and comfort to ease breathing
- Working within agreed care plans and clinical guidance
Daily Living Support

Monitoring & Early Escalation
- Observing changes in breathing or condition
